Admission Requirements
- Application Form & Medical Evaluation (within 30 days)
- Chest X-Ray or TB Test (within 6 months)
- History and Physical, Nurse Notes, Medication List
- Flu, Pneumococcal and COVID Declarations
- Insurance or TennCare Documentation
- Emergency Contacts
Once we receive the above information we will schedule an assessment with our nurse.
